You can’t improve what you don’t measure. Yet many clinic owners only look at one number, the money in the bank at month end. Tracking a small set of key performance indicators (KPIs) shows you where revenue comes from, where time is lost and which patients are drifting away. Here are twelve metrics worth watching, grouped by area.

Financial KPIs

1. Monthly revenue

Total income collected in the month. Track the trend over at least 12 months to see growth and seasonality.

2. Revenue per doctor

Revenue attributed to each doctor. In multi-doctor clinics this shows performance and supports fair revenue-sharing.

3. Average revenue per visit

Monthly revenue ÷ number of completed visits. A falling average can signal more discounting or a shift towards lower-value visits.

4. Discounts as a share of revenue

Total discounts ÷ gross revenue. Discounts are easy to give and easy to forget. Watching this number keeps them intentional.

5. Operating expenses and profit

Track rent, salaries, supplies and software, then subtract from revenue to see true profit. Revenue growth means little if costs grow faster.

Patient flow KPIs

6. Appointments booked

Total bookings per month, by doctor and by visit type. This is your leading indicator for future revenue.

7. No-show rate

Missed appointments ÷ total booked appointments. Every no-show is lost time and revenue. If this number is high, start with automated reminders. Our guide to reducing no-shows has ten more ideas.

8. Schedule utilisation

Booked hours ÷ available hours. Low utilisation means idle capacity; very high utilisation can mean long waits and burnt-out staff.

9. Peak hours

When are you busiest? A heatmap of appointments by day and hour shows when to add front-desk support or extend a doctor’s hours.

Patient KPIs

10. New patients per month

How many first-time patients you see. This shows whether your marketing, referrals and online presence are working.

11. Patient retention

The share of patients who return within an expected period. Patients who don’t come back for a follow-up may need a reminder, or may have had a poor experience.

12. Patient demographics

Age, gender and location mix help you plan services, opening hours and marketing that match who you actually serve.

How often should you review KPIs?

  • Daily: appointments booked and completed, no-shows.
  • Weekly: utilisation and peak hours.
  • Monthly: revenue, revenue per doctor, expenses, new patients and retention.

Pick three or four numbers to discuss with your team each month, agree one action for each, and check the result the following month.

Stop building spreadsheets

Most of these KPIs are already hidden in your appointment book and billing records. Clinic billing and finance software with built-in analytics calculates them automatically: monthly revenue trends, per-doctor performance, demographics and peak-hour heatmaps, plus PDF reports for your accountant.
